Summary:

The Children's Mental Health Targeted Case Manager provides strengths-based, culturally responsive case-management services to children and youth diagnosed with a Serious Emotional Disturbance (SED). The role supports families in accessing mental health, educational, medical, social, and community resources. This position focuses on building stability at home, school, and in the community through assessment, coordination, advocacy, and ongoing monitoring.

 

Essential Duties and Responsibilities:

  • Carry a caseload of 7-8 children/families
  • Conduct initial and ongoing functional assessments for children with SED.
  • Review diagnostic assessments and gather relevant background information.
  • Determine program eligibility and complete all intake paperwork and releases.
  • Develop an Individual Family Community Support Plan (IFCSP) within regulatory timelines.
  • Identify mental health, developmental, educational, social, and environmental needs.
  • Collaborate with the child, family, and treatment providers to establish measurable goals.
  • Connect families to appropriate services, including therapy, psychiatry, skills work, educational supports (IEP/504), social services, housing supports, and community programs.
  • Facilitate communication between the family, school, county, and other providers.
  • Support families navigating system complexities (insurance, benefits, school meetings, etc.).
  • Maintain regular contact with clients (often monthly minimum) through home, school, office, or community visits.
  • Track progress toward goals and adjust service plans as needed.
  • Ensure services are delivered in accordance with the treatment plan and state guidelines.
  • Advocate for the child's needs within schools, healthcare systems, and community settings.
  • Support families in understanding their rights and accessing culturally responsive services.
  • Assist with crisis planning and connect families to crisis resources when needed.
  • Support transitions such as hospital discharge, residential treatment return, or placement changes.
  • Maintain accurate and timely case notes, assessments, IFCSPs, and billing documentation.
  • Meet all state, county, and agency requirements for CMH-TCM services.
  • Participate in team meetings, case consultations, and required training.
  • Other related duties may be assigned.

 

Qualifications/Experience:

  • Spanish speaking preferred.
  • Bachelor's degree in Behavioral Science, Social Work, Psychology, Sociology, or a related field.
  • Experience working with children or youth with mental health or behavioral needs.
  • Strong communication skills and ability to collaborate across systems.
  • Valid driver's license and reliable transportation for community-based work.
  • Ability to work with diverse families and maintain professional boundaries.
  • Experience with electronic health records or case-management documentation.
  • Bilingual or lived experience supporting underrepresented communities.
